New Starter Questionnaire

What is a New Starter Questionnaire (NSQ) ?

Also known as pre-placement questionnaires, a New Starter Questionnaire is a form an employer issues (typically through Occupational Health) once an offer of employment has been made and accepted by an employee.

So what does the process entail and what are the benefits?

For the employer: 

This is an excellent opportunity to give employees the ability to bring forward any health concerns or conditions to Occupational Health that may impact on their working ability. Production of a fitness to work certificate is issued based on the evidence disclosed and any discussion with the Clinician.

The process is fast, Valentine offers a secure paperless new starter screening process, often completed within 48 hours.

The service will aid the business in making key decisions and risk assessing an individual and their ability to work in the role from a health perspective.

Lastly, the cost of screening employees is extremely cost effective from as little as £20 + vat.

For the employee:

The process is quick and easy and can often be completed by answering just a few short questions. You may not need to speak to an OH Clinician verbally as the process is typically electronic.

Employees are given an opportunity to share any health concerns that may impact on the new role they are commencing. Advice is then fed back to HR in an appropriate method with tailored guidance.

Our role:

As the Independent OH Provider, our aim is to screen employee’s fitness for work. Occasionally a call may be required to the individual to discuss the content of their questionnaire and the role further.  We then provide feedback given as part of the fitness to work certificate which may be ‘fit for work’, ‘fit for work with restrictions or adjustments’ or ‘not fit for work’.
